



Required Documents
Deadline: March 1 (each year)*
Applicants must submit the following documents to be considered for a
scholarship:
- Completed Scholarship Application
- Completed Questionnaire
- Letter of Recommendation from a
person who is not a relative. Letter may be from a teacher, employer or individual who is familiar with the student's
personal abilities and goals.
- Sealed Official School Transcripts or GED.
This document must be sealed -- no copies, faxes, or Internet
transcripts. Students who have not yet attended college must submit official
high school transcripts or GED with a Cumulative GPA.
- Clean copy of Sealaska Shareholder Identification
Card or a copy of the stock certificate. If applying as a lineal
descendant of a shareholder, applicant must provide a copy of
parent/grandparent's Sealaska shareholder ID card or stock certificate.
- A clean copy of student's Certificate of Indian
Blood or Tribal Enrollment Card. If student does not have either
one or must request one from the BIA, for example, applicant should fill out
the BIA request form and send a copy to SHI with the scholarship application.
The copy of the BIA request form shows proof the student has ordered the
Certificate of Indian Blood or Tribal Enrollment Card. Once applicant receives
the certificate or card, a clean copy should be sent to our office

- A clean copy of student's Birth Certificate.
No birth cards, must show parents' names. If qualifying through a
grandparent, applicant must turn in a clean copy of the parent's birth
certificate and the student's birth certificate to prove the applicant is a
lineal descendant.
- School Admission/Acceptance Letter
from the school student plans to attend. This is where scholarship checks will
be mailed. This must be postmarked on or before July 15.
* School Admission/Acceptance Letters are an exception to the March 1 deadline.
